This is the official app for the 12th European Peritoneal Dialysis Meeting, 2-5 Oct 2015, Krakow, Poland The theme of this year's meeting is 'Knowledge in PD is the Key'. During the meeting we will explore all facets of the care of end stage renal disease patients who wish to take control of their own lives and be treated at home. We will focus on how the multidisciplinary team can best work together to ensure that we put patients at the forefront of care, supporting them with informed choices, better information and giving them more control over their treatment. The topics will cover the basic medical and scientific aspects of peritoneal dialysis but will also provide a holistic approach to the care of patients.
